Contact us within 31 days of leaving an Employer Benefits Plan. We'll transition your Health, Prescription Drugs, Dental and Travel benefits into a new Conversion Plan, with no medical questionnaire.

You can personalize your Conversion Personal Health Plan to meet your specific needs by including the following benefits.

Prescription Drugs

  • 80% coverage of prescribed drugs listed in the Saskatchewan Prescription Drug Plan Formulary, up to $500 per person per contract year

Dental Plan

  • First contract year — 75% for Basic, maximum of $500 per person
  • Second contract year — 80% Basic & 50% Major, overall maximum of $750 per person
  • Third and following contract years — 80% Basic & 50% Major, overall maximum of $1,000 per person per year 

Waiting period

  • If you had dental benefits for longer than 6 months at time of group plan termination, benefits will continue uninterrupted according to the number of years of coverage
  • If you had dental benefits for less than 6 months, the waiting period will be equivalent to 6 months less the time dental benefits were in effect on your group plan
  • If you group plan did not include dental benefits, a 6-month waiting period will apply.
  • Basic dental services include examinations, x-rays, tests, cleaning, fillings, root canals, oral surgery, denture repairs, etc.
  • Major dental services include inlays, onlays, crowns, dentures, bridges, and some orthodontic services

Hospital Cash

  • Provides financial help to meet expenses due to hospitalization
  • Under age 65 – $100 per day, up to 50 consecutive days in hospital
  • Age 65 and over – $100 per day, up to 30 consecutive days in hospital

VIP Annual Travel Plan 

  • Coverage up to 30 consecutive days per trip with no limit on the number of trips
  • $5,000,000 in emergency hospital and medical benefits while travelling outside of Saskatchewan, with no deductible
  • Travel Assistance is available 24 hours a day, 365 days a year
  • $100,000 in air flight and common carrier accident insurance
  • Up to $1,000 for trip interruption
  • Up to $1,000 for baggage and personal effects
  • Up to $1,500 for meals and accommodations if trip is delayed due to injury or illness

Critical Conditions

  • Provides a lump-sum cash payment to help cope with a severe critical illness or condition resulting from disease
  • Eligible conditions: severe heart attack, severe stroke, life-threatening cancer, Alzheimer's disease, coma, major organ failure, major organ failure requiring transplant, motor neurone disease, paralysis, senile dementia, blindness, deafness, loss of speech, Multiple Sclerosis, Parkinson's disease and severe burns
  • Choose from:
    • Level 1 – $10,000 for each Insured or spouse under age 65 and $5,000 for each dependent child
    • Level 2 – $25,000 for each Insured or spouse under age 65 and $10,000 for each dependent child

Term Life Insurance

  • $25,000 to $100,000 coverage
  • Apply up to age 55; renew up to age 70
  • Optional $10,000 coverage for each dependent child
